Commercially packed potato salad with past use by date - packaged commercially processed foods that are for sale to the consumer shall be removed from sale immediately after the manufacturer's expiration date or the sell-by-date. COS: Voluntarily discarded. Corrected On-Site. New Violation. No test kit for QUAT sanitizer being used for sanitizing. Need to keep QUAT sanitizer at 200ppm using test kit to verify. Repeat Violation. Restroom door does not have a self-closer. Need a door to the restroom that is tight-fitting and self-closing. COS: Self-closer has been purchased- keep self-closer on door. Corrected On-Site. New Violation.

The effective date of the new Food Service Rules and Regulations is February 15, 2006. Establishments permitted after February 15, 2006 will have 90 days to obtain certification for a food service manager. Enforcement of all other requirements begins on November 15, 2006.
